How often do you change your workout routine?

I recently caught myself in a situation when I see that I did not change my basic workout plan for last 8-12 months, just some minor changes with exercises where I replace barbells and dumbells and some new exercises here and there.

How about you ?

How often you change it and if you change what you change and how?

I often change my routines every 3-4 months or if I no longer notice any new gains or improvements.
